Most sous vide steak guides give you two numbers: a temperature and a time. They are usually right about the temperature and vague about the time, and they almost never mention the third number that actually determines whether the steak is safe.
Here is the complete picture.

Temperature sets doneness, and nothing else does
This is the part sous vide makes genuinely easy. In a water bath the steak cannot get hotter than the water, so the bath temperature is the final internal temperature. No carryover to guess at, no window to hit.

Ribeye and strip take heat slightly better than lean cuts because their fat needs a bit of warmth to render pleasantly. Many cooks run ribeye a couple of degrees above where they would run a filet for that reason.
Thickness sets the time, not weight
This is where most guides go wrong, and it is the single most useful thing to understand about sous vide.
Heat travels into a steak from both faces. How long that takes depends on how far it has to travel, which is thickness, and has essentially nothing to do with how much the steak weighs. A two-inch steak takes the same time whether it weighs twelve ounces or two pounds. A wide, thin steak and a narrow, thin steak of the same thickness finish together.
The relationship is not linear either. Doubling the thickness roughly quadruples the time. That is why a one-inch steak is done in about an hour and a two-inch steak needs closer to three.
Measure the thickest point with a ruler, not by eye. The third number: pasteurization
Here is the thing most guides skip.
"Time to core temperature" and "time to safe" are two different numbers. The first makes the steak hot. The second makes it safe. For a steak that has been cooked conventionally these collapse into one question, because 145°F kills pathogens fast enough that hitting the temperature is effectively the same as being safe. At sous vide temperatures they come apart.
Pasteurization is a function of temperature and time together. Hold food at a lower temperature for longer and you achieve the same reduction in pathogens as a higher temperature held briefly. This is not a loophole. It is how commercial food safety has worked for a century, and it is why sous vide chicken at 145°F is a real thing rather than a reckless one.
The published tables, from Douglas Baldwin's widely cited guide, give times that achieve at least a million-to-one reduction in Listeria and a ten-million-to-one reduction in Salmonella. For a 25 mm steak:

The 130°F problem
Now the uncomfortable part. 130°F is the most popular medium-rare sous vide temperature and it pasteurizes nothing.
No published pasteurization table covers 130°F or below, because at those temperatures you are not meaningfully reducing pathogens no matter how long you hold. The tables for beef start at 131°F, and that one degree is not arbitrary.
This does not make a 130°F steak reckless for a healthy adult. It makes it equivalent to a rare steak off a grill, which millions of people eat happily. But it does come with rules:
- Treat it as unpasteurized. It should go fridge to plate within about four hours, total.
- Do not hold it in the bath for hours "because sous vide is forgiving." That forgiveness is a property of pasteurized food.
- Do not serve it to anyone pregnant, elderly, very young, or immune compromised.
- If you want the same colour with an actual safety margin, run 131°F to 133°F instead. The visual difference is almost nothing.

Times for common thicknesses
Starting from fridge temperature, in a 131°F bath, for a slab-shaped steak:

Note that a 1 inch steak is 25.4 mm, which means it should be treated as the 30 mm row rather than the 25 mm one. Rounding down shortens the time, and shortening the time is the one direction you never want to be wrong in.
From frozen
You can drop a frozen steak straight into the bath. It works and it is one of the genuine conveniences of the method.
A frozen steak takes roughly 50 percent longer to bring its core up than a refrigerated one. Because pasteurization only starts counting once the core is at temperature, the longer heating time usually covers it. Set the calculator to frozen and it uses the frozen heating table and tells you what it assumed.
How long is too long?
Sous vide is forgiving on time but not infinitely so. The bath cannot overcook the steak in the usual sense, because it cannot get hotter than the set temperature. What it can do is change the texture.
Past about four hours, tender cuts start to soften in ways most people do not enjoy. The muscle fibres break down, and a ribeye that spent eight hours at 131°F comes out mushy rather than tender. The steak is still safe. It is just not a steak anymore.
Tough cuts are the opposite. Chuck and short rib genuinely benefit from 24 to 48 hours, because they have the connective tissue to spare. That is a different technique with a different goal.
Searing: the step that makes it a steak
Sous vide produces a steak that is perfectly cooked and grey. The crust is entirely on you, and it is not optional.
Three things matter.
Dry it. Take the steak out of the bag and pat it completely dry with paper towels. Any surface moisture has to boil off before browning starts, which wastes time and steams the surface. Some cooks rest the steak uncovered in the fridge for ten minutes.
Get the pan hotter than feels reasonable. Cast iron, ripping hot, with a high smoke point oil. You want the crust to form in 45 to 60 seconds per side. Anything slower and the heat works its way inward and puts a grey band around the edge, which defeats the point.
Keep it short. Sixty seconds a side, maybe a quick edge sear. A cast iron skillet holds heat better than stainless and recovers faster when the cold steak hits it. A propane searing torch is the other popular route and avoids the grey band entirely.
Do not rest the steak after searing. It already rested for hours in the bath, the interior is already at a uniform temperature, and there is nothing to redistribute. Slice it and eat it.
Equipment
A circulator. Any of the mainstream sous vide immersion circulators hold temperature well enough. Prices have come down enough that the cheap ones are no longer a meaningful compromise on accuracy.
Bags. A vacuum sealer is convenient but not required. The water displacement method with a zip-top bag works: lower the open bag into the water and let pressure push the air out before sealing.
Something to stop it floating. A floating bag means part of the steak is not in contact with circulating water, which invalidates every number in this guide. Clip it to the side or weigh it down.
The bottom line
Pick your temperature for doneness. Measure thickness to get the time. Check the pasteurization number if you care about safety margin, and know that 130°F gives you none. Dry the steak hard, sear it fast, and skip the rest.
